The MCL is macroscopically intact, but there is overlying bright signal (arrowheads), reflecting superficial edema associated with the presence of torn fibers. In Grade 1 sprains, the edema is seen only superficially. The MCL is of normal thickness and is closely applied to the cortical bone.
