Double set-up refers to simultaneous preparation for both direct oral intubation and emergency cricothyrotomy.

EGD: extraglottic airway device; IV: intravenous catheter; PTV: percutaneous transtracheal ventilation; RSI: rapid sequence intubation; SaO2: arterial oxygen saturation.

* "Triple setup" refers to simultaneous preparation for direct tracheal intubation, a nonsurgical rescue device (eg, an EGD) if appropriate, and emergency cricothyrotomy.

¶ Signs of significant airway or vascular injury include stridor, drooling, trismus, tracheal deviation, crepitus, expanding hematoma, thrills, bruits, and neurologic deficit consistent with cerebral ischemia.
