Patterns of platelet refractoriness

Patterns of platelet refractoriness
The figure shows the typical patterns of response to platelet transfusion.
  • Solid line – Normal platelet survival. The platelet count increases after transfusion and remains elevated, with a gradual decline by approximately 3 days.
  • Dashed line – Typical pattern for a non-immune mechanism of platelet refractoriness. The initial platelet count increment measured 10 minutes to 1 hour after platelet transfusion is relatively normal, but the decline in platelet count is accelerated, with the count at 24 hours similar to baseline.
  • Dotted line – Typical pattern for alloimmune refractoriness. The initial platelet count increment measured 10 minutes to 1 hour after platelet transfusion fails to increase appreciably.

Refer to UpToDate for further information about evaluation and management of platelet refractoriness.
