Patients admitted to PICU

Doubling of serum Cr

OR

eCrCl decrease >50 percent

OR

ICU fluid overload >15 percent
High-risk patients

Acute decompensated heart failure

Stem cell transplant recipient

Serum Cr increase ≥0.3 mg/dL

OR

eCrCl decrease 25 to 50 percent

OR

ICU fluid overload >10 percent
Very-high-risk patients
Receiving mechanical ventilation and one or more vasoactive medications

Any serum Cr increase

OR

eCrCl decrease >25 percent

OR

ICU fluid overload >5 percent
