Clinical feature Rationale
Occurrence at random time of the night (rather than first one-third of the night as in parasomnias associated with arousal from NREM sleep*)
  • These features suggest the possibility of nocturnal seizures, rather than parasomnias
  • For children with these symptoms, we recommend including a 16-channel EEG with the polysomnogram to investigate the possibility of nocturnal seizures
Very frequent occurrence (>2 or 3 nights per week)
Hypermotor thrashing behavior during nocturnal event
Short duration of event (seconds, rather than minutes as in arousal parasomnias)
Fatigue upon awakening This feature suggests obstructive sleep apnea or possibly nocturnal seizures
NREM: non-rapid eye movement; EEG: electroencephalogram.
* The most common parasomnias in children are sleepwalking, confusional arousals, and sleep terrors, which occur upon partial arousal from NREM sleep.
