Retrobulbar hemorrhage with orbital compartment syndrome
This patient had acute onset of decreased vision and periorbital swelling and hemorrhage. Physical examination demonstrated markedly decreased vision, an afferent pupillary defect, proptosis, subconjunctival hemorrhage, chemosis, ophthalmoplegia, tightness of the eyelids, and decreased retropulsion (resistance to attempts to push the eye farther back into the orbit).
