Initial prescription for urgent-start peritoneal dialysis
Volume per exchange and number of exchanges:
  BSA ≤1.7/m2 BSA >1.7/m2
If eGFR >7 mL/min/1.73 m2 750 mL four cycles 1000 mL five cycles
If eGFR ≤7 mL/min/1.73 m2 1000 mL six cycles 1250 mL six cycles
Total cycler time:
Four cycles - Minimum of 5 hours
Five cycles - Minimum of 6 hours, 40 minutes
Six cycles - Minimum of 8 hours (staffing and time permitting)
Solution:
1.5% dextrose dialysate, if no peripheral edema or shortness of breath
2.5% dextrose dialysate, if edema or shortness of breath
4.25% dextrose dialysate, if prescribed by treating physician
Special instructions:
All exchanges are to be performed with the use of a cycler in supine position
Have patient use bathroom before being connected to cycler
The patient should remain supine while dwelling any fluid
If the patient needs to sit up or stand, he/she will have to be fully drained
If patient has a cough, call clinician for a cough suppressant
Tell patients to avoid straining; if constipated, call clinician for laxative
Instruct patient to wear loose clothing
Eating during dialysis should be limited to the drain phase of exchange
If patient has severe volume overload, call clinician for high-dose oral diuretic and 4.25% dextrose dialysate
eGFR as per MDRD equation.
IPD: intermittent peritoneal dialysis; eGFR: estimated glomerular filtration rate; BSA: body surface area.
