Quadriceps contusion rehabilitation scheme
  Phase 1 Phase 2 Phase 3
Purpose
  • Limit hemorrhage
  • Restore pain-free motion
  • Functional rehabilitation
  • Regain strength and endurance
Treatments
  • Weight bearing as tolerated with crutches
  • Leg elevation
  • Ice massage 10 to 15 minutes 3 to 10 times per day
  • Cold pack or cold whirlpool 20 minutes
  • Full thigh compression wrap
  • Isometric contractions of 8 to 10 reps 3 to 5 times per day as tolerated
  • Ice or cold therapy 15 to 20 minutes 3 to 5 times per day
  • Active flexion while supine and prone
  • Low resistance cycling
  • Restoration of normal weight bearing gait
  • Discard crutches when >90 degrees knee flexion, no limp, pain free weight bearing gait
  • Cycling
  • Running
  • Swimming
  • Functional exercises
  • Increase in activity intensity OR volume no more than 10% per week
Indications for advancing to next phase
  • Pain free at rest
  • Thigh girth no longer increasing
  • >120 degrees pain free active knee flexion
  • Equal thigh girth
  • Full active range of motion
  • Perform all activities pain-free
  • Thigh pad with contact sports for next 3 to 6 months
