Score index for radiosurgery (SIR) in brain metastases

Score index for radiosurgery (SIR) in brain metastases
Variable Points
0 1 2
Age (yrs) ≥60 51 to 59 <50
KPS ≤50% 60 to 70% 80 to 100%
Systemic disease status Progressive Stable NED
Number of CNS lesions ≥3 2 1
Volume of treated lesions ≥13 cm3 5 to 13 cm3 <5 cm3
Total score (0 to 10 points):
The modified SIR score is calculated as the addition of partial scores obtained from five prognostic factors: Age, KPS, extracranial disease status, number of brain metastases, and volume of the largest brain metastasis.
Poor risk = 0 to 4 versus good risk = 5 to 10.
KPS: Karnofsky performance status; NED: no evidence of disease; CNS: central nervous system.
