Adverse Reactions

Cardiovascular: Flushing, rapid pulse

Central nervous system: Dizziness, drowsiness

Gastrointestinal: Fecal discoloration (blue to blue-green), nausea, vomiting, xerostomia

Genitourinary: Acute urinary retention, difficulty in micturition, urine discoloration (blue to blue-green)

Hypersensitivity: Hypersensitivity reaction (rare)

Ophthalmic: Blurred vision

Respiratory: Dyspnea

Contraindications

Hypersensitivity to hyoscyamine, methenamine, methylene blue, sodium phosphate monobasic, or any component of the formulation. Note: Contraindications to methenamine and hyoscyamine may also apply; see Methenamine and Hyoscyamine individual monographs for more information.

Warnings/Precautions

Concerns related to adverse effects:

• Belladonna alkaloid allergy: Use with caution in patients with a history of intolerance to belladonna alkaloids.

• Blurred vision: Discontinue use immediately if blurred vision occurs.

• Dizziness: Discontinue use immediately if dizziness occurs.

• Salicylate allergy: Use with caution in patients with a history of intolerance to salicylates.

• Tachycardia: Discontinue use immediately if tachycardia occurs.

Disease-related concerns:

• Cardiovascular disease: Use with caution in patients with cardiovascular disease (cardiac arrhythmias, congestive heart failure, coronary heart disease, mitral stenosis).

• Gastrointestinal disease: Use with caution in patients with gastrointestinal tract obstruction or gastric ulcers.

• Glaucoma: Use with caution in patients with glaucoma.

• Myasthenia gravis: Use with caution in patients with myasthenia gravis.

• Obstructive uropathy: Use with caution in patients with obstructive uropathy (bladder neck obstruction or prostatic hyperplasia).

Special populations:

• Pediatric: Not recommended for use in children ≤6 years of age.

Beers Criteria: Hyoscyamine is identified in the Beers Criteria as a potentially inappropriate medication to be avoided in patients 65 years and older (independent of diagnosis or condition) due to its highly anticholinergic properties and uncertain effectiveness as an antispasmodic (Beers Criteria [AGS 2023]).

Antacids: May decrease therapeutic effects of Methenamine. Management: Consider avoiding the concomitant use of medications that alkalinize the urine, such as antacids, and methenamine. Antiemetics (5HT3 Antagonists): May increase serotonergic effects of Serotonergic Agents (High Risk). This could result in serotonin syndrome. Benzhydrocodone: May increase serotonergic effects of Monoamine Oxidase Inhibitors. This could result in serotonin syndrome. Management: The use of benzhydrocodone is not recommended for patients taking monoamine oxidase inhibitors (MAOIs) or within 14 days of MAOI discontinuation. If coadministration is required, use test doses and frequent titration of small benzhydrocodone. Risk D: Consider Therapy Modification

Cyclobenzaprine: May increase serotonergic effects of Monoamine Oxidase Inhibitors. This could result in serotonin syndrome. Risk X: Avoid

Cyproheptadine: May decrease serotonergic effects of Monoamine Oxidase Inhibitors. Monoamine Oxidase Inhibitors may increase anticholinergic effects of Cyproheptadine. Risk X: Avoid

Dapoxetine: May increase serotonergic effects of Serotonergic Agents (High Risk). This could result in serotonin syndrome. Management: Do not use serotonergic agents (high risk) with dapoxetine or within 7 days of serotonergic agent discontinuation. Do not use dapoxetine within 14 days of monoamine oxidase inhibitor use. Dapoxetine labeling lists this combination as contraindicated. Risk X: Avoid

DOPamine: Monoamine Oxidase Inhibitors may increase hypertensive effects of DOPamine. Management: Initiate dopamine at no greater than one-tenth (1/10) of the usual dose in patients who are taking (or have taken within the last 2 to 3 weeks) monoamine oxidase inhibitors. Monitor for an exaggerated hypertensive response to dopamine. Risk D: Consider Therapy Modification

Tapentadol: May increase adverse/toxic effects of Monoamine Oxidase Inhibitors. Specifically, the additive effects of norepinephrine may lead to adverse cardiovascular effects. Typhoid Vaccine: Antibiotics may decrease therapeutic effects of Typhoid Vaccine. Only the live attenuated Ty21a strain is affected. Management: Avoid use of live attenuated typhoid vaccine (Ty21a) in patients being treated with systemic antibacterial agents. Postpone vaccination until 3 days after cessation of antibiotics and avoid starting antibiotics within 3 days of last vaccine dose. Risk D: Consider Therapy Modification

Tyrosine: May increase hypertensive effects of Monoamine Oxidase Inhibitors. Risk X: Avoid

Umeclidinium: May increase anticholinergic effects of Agents with Clinically Relevant Anticholinergic Effects. Risk X: Avoid

Uva Ursi: Urinary Acidifying Agents may decrease therapeutic effects of Uva Ursi. Management: Consider avoiding use of uva ursi with agents that acidify the urine, as this may impair uva ursi efficacy. Risk D: Consider Therapy Modification

Valbenazine: May increase adverse/toxic effects of Monoamine Oxidase Inhibitors. Risk X: Avoid

Pregnancy Considerations

Animal reproduction studies have not been conducted with this combination. Hyoscyamine and methenamine cross the placenta. Information related to the oral use of methylene blue in pregnancy is limited (Heinonen 1977).

Breastfeeding Considerations

Hyoscyamine and methenamine can be detected in breast milk. The manufacturer recommends that caution be used if administered to a nursing woman.

Dietary Considerations

Foods/diets which alkalinize urine pH >5.5 decrease activity of methenamine.

Mechanism of Action

Hyoscyamine: Hyoscyamine is a parasympatholytic that relaxes smooth muscles, producing an antispasmodic effect.

Methenamine: Methenamine is hydrolyzed in acidic urine to produce formaldehyde, which provides local bactericidal or bacteriostatic action.

Methylene blue: Methylene blue has weak antiseptic properties.

Sodium phosphate monobasic: Sodium phosphate monobasic maintains an acid pH in the urine, necessary for the degradation of methenamine.
