Tinea infections

Tinea infections:

Tinea corporis/tinea cruris : Topical: Cream 1%, solution 1%: Apply to affected and surrounding area(s) 1 to 2 times daily until clinical resolution, typically 1 to 3 weeks (Goldstein 2021; van Zuuren 2015; manufacturer’s labeling).

Tinea pedis (labeled use)/tinea manuum (off-label use): Topical: Cream 1%: Apply to affected and surrounding area(s) twice daily until 1 week after clinical resolution, typically for 4 weeks total (Goldstein 2021; Weinstein 2002; manufacturer’s labeling).

Tinea versicolor (pityriasis versicolor): Topical: Cream 1%, solution 1%: Apply a small amount to affected area(s) and surrounding skin once or twice daily for 3 weeks (Tanenbaum 1984; Tham 1987; manufacturer’s labeling).

Dosage adjustment for concomitant therapy: Significant drug interactions exist, requiring dose/frequency adjustment or avoidance. Consult drug interactions database for more information.

Administration: Adult

For external use only; avoid contact with the eyes. Apply a small amount and gently massage into affected and surrounding skin areas.

Use: Labeled Indications

Fungal infections:

Cream: Treatment of tinea pedis (athlete's foot), tinea cruris, and tinea corporis caused by Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, and Microsporum canis; treatment of tinea versicolor (pityriasis versicolor).

Solution: Treatment of tinea cruris and tinea corporis caused by Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, and Microsporum canis; treatment of tinea versicolor.

Limitations of use: Effectiveness has not been proven in tinea pedis (athlete's foot).

Use: Off-Label: Adult

Metabolism/Transport Effects

None known.

Drug Interactions

There are no known significant interactions.

Pregnancy Considerations

Adverse events have been observed in animal reproduction studies with large doses administered orally. Systemic absorption is limited following topical administration.

Breastfeeding Considerations

It is not known if sulconazole is present in breast milk. The manufacturer recommends that caution be exercised when administering sulconazole to breastfeeding women.

Mechanism of Action

Substituted imidazole derivative which inhibits metabolic reactions necessary for the synthesis of ergosterol, an essential membrane component. The end result is usually fungistatic; however, sulconazole may act as a fungicide in Candida albicans and Candida parapsilosis during certain growth phases.
