Longitudinal change in c-terminal fibroblast growth factor 23 and outcomes in patients with advanced chronic kidney disease

Helen V. Alderson, Rajkumar Chinnadurai, Sara T. Ibrahim, Ozgur Asar, James P. Ritchie, Rachel Middleton, Anders Larsson, Peter J. Diggle, Tobias E. Larsson & Philip A. Kalra

doi : 10.1186/s12882-021-02528-2

BMC Nephrology volume 22, Article number: 329 (2021)

Fibroblast growth factor23 (FGF23) is elevated in CKD and has been associated with outcomes such as death, cardiovascular (CV) events and progression to Renal Replacement therapy (RRT). The majority of studies have been unable to account for change in FGF23 over time and those which have demonstrate conflicting results. We performed a survival analysis looking at change in c-terminal FGF23 (cFGF23) over time to assess the relative contribution of cFGF23 to these outcomes.

The risk of chronic kidney disease in relation to anthropometric measures of obesity: A Swedish cohort study

Ensieh Memarian, Peter M. Nilsson, Isac Zia, Anders Christensson & Gunnar Engström

doi : 10.1186/s12882-021-02531-7

BMC Nephrology volume 22, Article number: 330 (2021)

It has been shown that individuals with obesity have a higher risk for chronic kidney disease (CKD). However, it is unclear which measure of obesity is most useful for prediction of CKD in the general population. The aim of this large prospective study was to explore the association between several anthropometric measures of obesity, i. e., body mass index (BMI), waist circumference (WC), waist circumference to height ratio (WHtR), waist-to-hip ratio (WHR), percentage of body fat (BF%), weight, height and incidence of hospitalizations due to CKD, in a population-based cohort study.

Improvements in six aspects of quality of care of incident hemodialysis patients – a real-world experience

Maciej Drozdz, João Frazão, Fatima Silva, Partha Das, Werner Kleophas, Wisam Al Badr, Szymon Brzosko & Stefan H. Jacobson

doi : 10.1186/s12882-021-02529-1

BMC Nephrology volume 22, Article number: 333 (2021)

The transition from chronic kidney disease stage 5 to initiation of hemodialysis has gained increased attention in recent years as this period is one of high risk for patients with an annual mortality rate exceeding 20%. Morbidity and mortality in incident hemodialysis patients are partially attributed to failure to attain guideline-based targets. This study focuses on improvements in six aspects of quality of dialysis care (adequacy, anemia, nutrition, chronic kidney disease-mineral bone disorder (CKD-MBD), blood pressure and vascular access) aligning with KDIGO guidelines, during the first 6 months of hemodialysis.

Oral sodium bicarbonate in people on haemodialysis: a randomised controlled trial

Stella I. Kourtellidou, Damien R. Ashby & Lina R. Johansson

doi : 10.1186/s12882-021-02549-x

BMC Nephrology volume 22, Article number: 346 (2021)

Adverse events and mortality tend to cluster around dialysis sessions, potentially due to the impact of the saw-toothed profile of uraemic toxins such as potassium, peaking pre-dialysis and rapidly dropping during dialysis. Acidosis could be contributing to this harm by exacerbating a rise in potassium. The objectives of this study were to investigate the effects of oral bicarbonate treatment on reducing inter-dialytic potassium gain as well as other clinical consequences of preserving muscle mass and function and reducing intradialytic arrhythmia risk in people on haemodialysis.

This study aims to determine the association of the positivity of IgG and the intensity of IgG staining with a poor renal prognosis.

doi : 10.1186/s12882-021-02567-9

BMC Nephrology volume 22, Article number: 358 (2021)

Alport Syndrome and IgA Nephropathy (IgAN) are both disorders that can cause hematuria. Alport syndrome is most commonly an X-linked disease, caused by COL4A5 mutation. Mutations of COL4A3 and COL4A4 on chromosome two are also common causes of Alport syndrome. IgAN is the most common glomerulonephritis worldwide. Though IgAN is usually sporadic, an estimated 15% of cases have an inheritable component. These cases of Familal IgA Nephropathy (FIgAN) can have mutations on genes which are known to cause Alport Syndrome.
