Imaging Fuchs Endothelial Corneal Dystrophy in Clinical Practice and Clinical Trials

Patel, Sanjay V. MD, FRCOphth

doi : 10.1097/ICO.0000000000002738

December 2021 - Volume 40 - Issue 12 - p 1505-1511

Corneal tomography has an emerging role in the assessment of Fuchs endothelial corneal dystrophy (FECD) in clinical practice and potentially for future clinical trials. Posterior elevation and pachymetry maps derived from elevation based Scheimpflug tomography can detect early corneal edema, even at a subclinical stage, enabling clinicians to better counsel patients about their vision and the risk of disease progression with and without cataract surgery. Tomographic imaging provides a functional assessment of corneal endothelial health, and could enable objective assessment of FECD progression, or regression, in response to novel therapeutic interventions. Clinicians and investigators should adopt Scheimpflug imaging for the assessment of FECD over traditional morphologic imaging modalities.

Cutting Edge: Novel Treatment Options Targeting Corneal Neovascularization to Improve High-Risk Corneal Graft Survival

Cursiefen, Claus MD, PhD*,†; Hos, Deniz MD, PhD*,†

doi : 10.1097/ICO.0000000000002736

December 2021 - Volume 40 - Issue 12 - p 1512-1518

Corneal neovascularization is an important risk factor for graft rejection after keratoplasty, although its role in posterior lamellar keratoplasty is not yet well defined. The aim of this work was to describe clinically available approaches that target corneal neovascularization preoperatively to improve graft survival after subsequent penetrating keratoplasty (PK) and to present findings on Descemet membrane endothelial keratoplasty (DMEK) in eyes with neovascularization.

Pellucid marginal degeneration (PMD) is a bilateral corneal ectatic disorder, characterized by thinning of the inferior cornea. Numerous surgical procedures have been described in the literature, and there is a lack of consensus regarding the best available surgical management. Recently, full-thickness intrastromal lamellar keratoplasty (ILK) was described as an option for treating PMD and emerged as a promising technique. However, ILK results in an irregular surface, leading to a significant residual against-therule (ATR) astigmatism.

Proctored surgical instruction has traditionally been taught through in-person interactions in either the operating room or an improvised wet lab. Because of the COVID-19 pandemic, live in-person instruction was not feasible owing to social distancing protocols, so a virtual wet lab (VWL) was proposed and implemented. The purpose of this article is to describe our experience with a VWL as a Descemet membrane endothelial keratoplasty (DMEK) skills-transfer course. This is the first time that a VWL environment has been described for the instruction of ophthalmic surgery.
