Nowadays, ultrasound-guidance is commonly used in regional anaesthesia (USGRA) and to locate the spinal anatomy in neuraxial analgesia. The aim of this second guideline on the PERi-operative uSE of UltraSound (PERSEUS-RA) is to provide evidence as to which areas of regional anaesthesia the use of ultrasound guidance should be considered a gold standard or beneficial to the patient. The PERSEUS Taskforce members were asked to define relevant outcomes and rank the relative importance of outcomes following the GRADE process.

Combined spinal-epidural anaesthesia (CSEA) using a needle-through-needle technique is currently widely used. However, successful epidural needle placement does not mean a successful spinal needle placement during CSEA. Whether ultrasound assistance could increase the first-pass success rate of spinal needle placement for CSEA remains unknown.

Hypoxaemia is frequently seen during flexible bronchoscopies that are done with a nasal approach under the traditional sedation with propofol. This study investigated the potential benefits of supraglottic jet oxygenation and ventilation (SJOV) using the Wei nasal jet tube (WNJ) in reducing hypoxaemia in patients undergoing bronchoscopy under moderate to deep intravenous sedation using a propofol, lidocaine and remifentanil cocktail.

International guidelines recommend cricothyroidotomy as a life-saving procedure for ‘cannot intubate, cannot ventilate’ situations. Although commercially available sets facilitate surgical cricothyroidotomy, regular training seems to be the key to success.

Videolaryngoscopy (VLS) has gained widespread popularity over the past few years, especially for management of difficult airways.1 However, even with VLS, intubation of the trachea can be challenging. For these situations a video-assisted fibreoptic intubation (VAFI) technique has been suggested.2 A VLS is used to obtain the best possible glottic view and a fibrescope preloaded with a tracheal tube is introduced to intubate the trachea.
