Medial meniscus posterior root tears alter tibiofemoral contact, leading to medial knee osteoarthritis. Repair can restore kinematics and biomechanics. Female sex, age, obesity, high posterior tibial slope, varus malalignment >5°, and Outerbridge grade ≥3 chondral lesions in the medial compartment are associated with risk for medial meniscus posterior root tears and poor healing after repair. Extrusion, degeneration, and tear gap may increase tension across the repair site, resulting in poor outcome.

Hip labral “width� should be defined as the distance from the chondrolabral junction to the tip of the labrum in triangular cross-section. “Height� should refer to the distance from the joint surface to the perilabral recess (perpendicular to the width). “Length� is the distance from the anterior end of the labrum, adjacent to the anterior transverse acetabular ligament (TAL), to the posterior end of the labrum, adjacent to the posterior TAL (as may be relevant in reconstructions).

Arthritis of the patellofemoral compartment affects up to 24% of women and 11% of men over the age of 55 years who have symptomatic osteoarthritis of the knee. Patellofemoral cartilage lesions have been associated with several different geometric measures of patellar alignment, including the tibial tubercle–trochlear groove (TTTG) distance, trochlear sulcus angle, trochlear depth, and patellar height.

Femoral and tibial tunnel locations for ACL grafts should be predicated on anatomy. Regarding femoral ACL socket or tunnel creation, multiple techniques have been debated. Network meta-analysis shows that the anteromedial portal (AMP) technique results in better anteroposterior and rotational stability than does the “standard� constrained, transtibial technique based on side-to-side differences in laxity and pivot-shift tests, as well as IKDC objective scores.

The anterior cruciate ligament (ACL) and medial meniscus both contribute to anteroposterior translation of the tibia. Biomechanical studies have found increased translation at both 30° and 90° when transecting the posterior horn of the medial meniscus, and clinically, medial meniscal deficiency has been shown to have a 46% increase in ACL graft strain at 90°.
