Hepatitis B related dilemmas in the renal unit

Stephen G. Holt Stephen Locarnini Joe Sasadeusz

doi : 10.1111/nep.13815

Volume 26, Issue 4, April 2021, Pages: 287-293

This study reviewed the issues pertinent to hepatitis B virus (HBV) infection in kidney failure patients, and made suggestions on HBV serologic testing and segregation decisions based on current understanding and evidence. Four case scenarios are presented to guide clinicians on the management approach.

Comparison of skin cancer awareness and sun protection behaviours between renal transplant recipients and patients with glomerular disease treated with immunosuppressants

Zaw Thet Alfred K. Lam Shu?Kay Ng Soe Yu Aung Thin Han Dwarakanathan Ranganathan Jennifer Borg Christine Pepito Tien K. Khoo

doi : 10.1111/nep.13840

Volume 26, Issue 4, April 2021, Pages: 294-302

This cross?sectional single centre study evaluated the risk of skin cancer and photo?protective behaviour in kidney transplant recipients and patients with glomerular disease exposed to chronic immunosuppression. The authors showed that despite the understanding that chronic exposure to immunosuppression predisposes to a higher risk of skin cancers, over one in five patients do not adhere to simple photo?protective behaviours suggesting the need for more education.

Modification of the effects of intensive systolic blood pressure control on kidney outcomes by baseline body mass index

Adhish Agarwal Guo Wei Robert Boucher Faris Ahmed Srinivasan Beddhu

doi : 10.1111/nep.13857

Volume 26, Issue 4, April 2021, Pages: 303-311

This post?hoc analysis of the systolic blood pressure intervention trial (SPRINT) Study demonstrated increased incidence of acute kidney injury and chronic kidney disease (CKD) with intensive blood pressure (BP) reduction across all levels of body mass index (BMI). In men but not in women, however, higher BMI was associated with increasing rates of CKD with intensive BP treatment. A higher baseline BMI was associated with an augmented risk of incident CKD with intensive systolic BP control.

Effects of transient acute kidney injury, persistent acute kidney injury and acute kidney disease on the long?term renal prognosis after an initial acute kidney injury event

Keitaro Nagata Taro Horino Yutaka Hatakeyama Tatsuki Matsumoto Yoshio Terada Yoshiyasu Okuhara

doi : 10.1111/nep.13831

Volume 26, Issue 4, April 2021, Pages: 312-318

Among 7528 Japanese patients from a single centre with at least two serum creatinine measurements between 1995 and 2016 who experienced AKI, the outcomes of transient (less than 48 hour), persistent AKI (recovered within 7?days) and acute kidney disease (AKD) groups (recovery after 7 and less than 90?days) were compared with 1000 randomly selected non?AKI controls. Renal functional decline was faster after an episode of AKI and the risk of progression to CKD increased with the duration of AKI.

Inpatient and outpatient nephrology management of critically ill patients with acute kidney injury

David G. Ransley Emily J. See Alice Mizrahi Ray Robbins Rinaldo Bellomo

doi : 10.1111/nep.13838

Volume 26, Issue 4, April 2021, Pages: 319-327

This article provided new insights for post?acute kidney injury (AKI) care after discharge from the intensive care units. Less than 10% of patients have been followed up while nearly 35% developed a major adverse kidney event (MAKE) at 1 year, calling for attention in providing nephrology follow?up after AKI.

Prolonged SARS?CoV?2 viral shedding in patients with chronic kidney disease

Eoin D. O'Sullivan Jennifer S. Lees Katie L. Howie Dan Pugh Keith A. Gillis Jamie P. Traynor Iain Macintyre Patrick B. Mark

doi : 10.1111/nep.13844

Volume 26, Issue 4, April 2021, Pages: 328-332

In this study, the authors investigated the time between admissions to SARS?CoV?2 virus?negative nasopharyngeal swabs in inpatients. They found that CKD patients had longer time to viral clearance than non?CKD patients, and concluded that longer CKD patients had longer time to viral clearance, affecting renal service delivery, discharge planning, and admission capacity. In addition, potential anxiety and stigma have a direct impact on the patient.

Association and predictive value of geriatric nutritional risk index, body composition, or bone mineral density in haemodialysis patients

Sonoo Mizuiri Yoshiko Nishizawa Toshiki Doi Kazuomi Yamashita Kenichiro Shigemoto Koji Usui Michiko Arita Takayuki Naito Shigehiro Doi Takao Masaki

doi : 10.1111/nep.13826

Volume 26, Issue 4, April 2021, Pages: 341-349

Lower geriatric nutritional risk index (GNRI) in chronic haemodialysis patients was associated with lower fat tissue index and bone mineral density, and independently predicted 2?year all?cause mortality. Combining GNRI with traditional risk factors (age, sex, diabetes mellitus, cardiovascular disease, use of vasopressors for dialysis?related hypotension, and C?reactive protein) improved mortality prediction by 16 percentage points.

Nicotiflorin attenuates cell apoptosis in renal ischemia?reperfusion injury through activating transcription factor 3

Lin Wang Chenyu Li Chen Guan Yue Zhang Chengyu Yang Long Zhao Hong Luan Bin Zhou Lin Che Yanfei Wang Wei Zhang Hui Zhang Xiaofei Man Wei Jiang Yan Xu

doi : 10.1111/nep.13841

Volume 26, Issue 4, April 2021, Pages: 358-368

The study suggests that nicotiflorin, the component from a Chinese herb, has protective effect in a mouse model of renal ischaemia/reperfusion injury. By using computational molecular docking analysis and RNA silencing, the authors identified that nicotiflorin ameliorates renal cell apoptosis via activation of transcription factor 3 (ATF3).
