Notes

*A Glasgow coma scale score of 15 requires all the following: Spontaneous eye opening; accurate orientation to person, place, and time; and appropriate motor responses to commands.
For this calculation, Respiratory rate derived points = 1 when the respiratory rate is ≥22 breaths/min, and 0 for slower rates. Systolic blood pressure derived points = 1 when the systolic blood pressure is ≤100 mmHg, and 0 for higher blood pressures.
