  • CLSI recommends that any Enterococcus spp, Staphylococcus spp, or beta-hemolytic Streptococcus spp which tests daptomycin nonsusceptible be referred to a reference laboratory for testing. Similarly, EUCAST recommends that any Staphylococcus spp or beta-hemolytic Streptococcus spp which tests daptomycin nonsusceptible be referred to a reference laboratory.
  • CLSI reference daptomycin dosing (for Enterococcus spp): tests susceptible: 6 mg/kg every 24 hours; tests susceptible dose dependent: 8 to 12 mg/kg every 24 hours.
  • EUCAST reference daptomycin dosing: standard dose: 4 mg/kg every 24 hours, high dose: 6 mg/kg every 24 hours.
