Indications for PrEP based on sexual risk
  Factors that put someone at highest risk for acquiring HIV Other factors associated with acquiring HIV
Anal sex (insertive or receptive), particularly male same-sex intercourse
  • Condomless sex with partner with HIV and viral load is >200 copies/mL or unknown
  • Condomless sex with multiple or anonymous sex partners
  • Condomless sex with a partner at high risk for HIV (eg, from high prevalence setting, uses injection drugs)
  • Has been diagnosed with an STI (eg, syphilis, gonorrhea, chlamydia, mpox)
  • All other condomless anal sex, except if in mutually monogamous relationships with a partner known to be HIV negative or to have HIV viral load <200 copies/mL
Vaginal sex (insertive or receptive)
  • Condomless sex with a partner with HIV and the viral load is >200 copies/mL or unknown
  • Condomless sex with partner(s) at high risk for HIV (eg, multiple/anonymous partners from regions with high prevalence of HIV, main partner who uses injection drugs)
  • Has been diagnosed with syphilis, gonorrhea, mpox
  • Multiple partners from regions with low HIV prevalence
  • Has been diagnosed with other STI (eg, chlamydia, trichomonas)
This table summarizes groups at increased risk for acquiring HIV based on sexual risk. We discuss PrEP with all of these patient groups, inquiring about recent (eg, within the last 12 months) and future risk. The strength of our recommendation for PrEP varies depending on the degree of risk.
