This figure summarizes an approach to evaluating adults with nonacute esophageal dysphagia for solids and/or liquids. Refer to UpToDate content on esophageal dysphagia for evaluation and management of patients with acute dysphagia related to food impaction or foreign body ingestion.

EGJ: esophagogastric junction; FLIP: functional lumen imaging probe; GERD: gastroesophageal reflux disease.

* A complex stricture has any of the following features:
  • Length ≥2 cm
  • Angulated or irregular surface
  • Severely narrowed (diameter ≤10 mm)
  • Associated with large hiatal hernia (>5 cm)
  • Associated with esophageal diverticulum

¶ If manometric findings are suspicious for external compression of the esophagus, perform barium esophagram if not done previously.

Δ An example of inconclusive manometric findings may be EGJ outflow obstruction in the absence of other supporting data (eg, imaging or FLIP).

◊ Some patients with inconclusive manometric findings may evolve to a well-defined diagnosis over time. Thus, observation with symptom monitoring may be an option for such patients.
